Description: By kind permission of Mr Harry Johnson and family.
Only one representative (DC/CP, Team Trainer, or nominated Parent) should complete this form on behalf of each qualified Team or Pair.
Section 1 must be completed by 1st August, including Team Trainer name and PELHAM number.
Section 2 requires full rider and pony details and should be completed by 11am 5th August for names to be included on hoodies.
Completion by these dates allows us to allocate any Additional Qualifier places and ensures names are submitted in time for Championship Hoodies.
Please note: While team or pony changes can be reported beyond this date, hoodie names cannot be amended after the submission deadline.
Paper Declaration Forms must still be handed in on the morning of your competition.
Please note, entries including horse/pony names need to be made by 11am on the 5th of August to be included on the commemorative clothing.
